Formats for PF experiment

The new User Support System has been effective for the beamtime after April 1, 2007. The submission of the forms listed below should be made through this system. Please refer to the webpage Documents to be submitted for details.

Letter of agreement for experiments at PF (Form 1)
Application form for radiation work at PF (Form 9)
Certification of acceptance for radiation work (Form 10)

Firstly, please fill out and submit "User Registration" in the menu of "User Entry Procedures". When it is accepted, a temporary password will be sent to you by Email. Then, you can login to the system to submit the necessary forms. The temporary password is valid for 48 hours, please change the password by yourself.

Regarding the forms listed below, please follow the each procedure (forms or on-line application).
